who have already booked my services,
or for those who are sure they are going
to ​be doing so.

Please fill out all the following fields. This information is what will guide me during
​the design of your Poetry Art Card.

Your Poetry Art Card design includes the
​following on the front of the card:
• Your name / your company name
• The title of your event (i.e: wedding)
• The location of your event
• The date of your event
• Illustration/border illustration to fit the theme/meaning of your event

​All text is hand lettered, and you may
choose from script, print, or a mix of both.

Your spot illustration or border will
reflect the theme/occasion of your event.

I ask that you provide the following information to help me create your
custom illustration!

Once I receive your information, I will begin working on your custom Poetry Art Card.

​When I have finished the design, I will send you a proof. One limited revision and two proofs are included in the base price, and I am committed to making you happy!


ADDITIONAL OPTIONS:

If you would like your illustration to feature a portrait, or any other specific reference imagery, please email me the reference images after completing this form.
There is an additional $65 flat fee per portrait.

If you wish to customize the reverse of the card, for a more detailed illustration or design, or for something other than a card (how bout a brochure? or an 8x10 full color illustration?) please contact me for a quote.
